The Effect of Anticoagulation Service on Patient Satisfaction and Hospital Profit

Abstract
The anticoagulation service (ACS) provides patient education, monitoring of hemorrhagic and thromboembolic complications and the adjustment of warfarin dosage to maintain prothrombin time within the therapeutic range, in order to provide effective and safe anticoagulation therapy. The patients receive explanations regarding the disease and the medication in detail ; thus the patient compliance, the understanding about their medication and their confidence of doctors and pharmacists will improve. The ACS also offers the adjustment of warfarin dosage, which increase the number of patients seen at the hospital by decreasing the physician¢¥s patient consultation time in the outpatient and the days of hospitalization in the inpatient. The purpose of this study was to show the impact of the ACS on patient satisfaction and hospital profit. This study investigated ; the difference of patient satisfaction between the ACS patient and the non-ACS patient, the effect on hospital profit based on consultation time and treatment income, the effect on days of hospitalization based on the hospital days of ACS patients. The estimation of patient satisfaction : In fourteen items questioning about the satisfaction on their physician, pharmacists and other personnels, the study group was significantly superior to the control group (p < 0.05). Also, in six items questioning about their disease and the understanding about the medicine being taken and the patient compliance, the study group was significantly superior to the control group (p < 0.05). The effect on hospital profit : The average patient consultation time was ranged from 2.5 minutes to 7.5 minutes, and the one day treatment income was range from 44,000 ti 116,000 depending on the physician. The patient consultation time per month would decrease by 31.6 hours. As a result, the number of patients increased by 489 per month and the treatment income increased by 47,409,500 won per month. Thus it would lead to the increase in hospital profit by 568,941,000 won per year indirectly. The effect on days of hospitalization : In the period of June, 1998, 41 patients were followed at the ACS after the hospitalization. On the basis of 5¡­7 days needed for INR stabilization on warfarin, the number of patients leaving the hospital within 5¡­7 days was 8¡­16 and the shortened hospital days was 32¡­46 days. In the conclusion, this study showed that the ACS was significantly improved patient satisfaction and hospital profit, therefore it has proved the usefulness of pharmaceutical care in the ACS.
